This is Mujintou Monogatari by KSS for the PC-9801, a floppy disk set with manual, map, and box.
The disks were tested up through the title/wake-up screen, but the game was not played through to the end, so full playability can't be guaranteed — as is common with floppy disks this old, some mold may be present. All text is in Japanese.
