This is the NEC PC Engine DUO-R console (TurboDUO, model PI-TG10), a Japan-region system. Per the title, it comes with a controller pad and a power supply.

The Hu-card lid is broken and the console has a lot of stains. It reads both Hu-card and CD-ROM2 software: Hu-card games play normally with normal sound output, and CD-ROM2 games also load and play with normal sound output. One official DUO-R controller pad is included and works normally. The power supply (AC adapter) is not an NEC official product, so its connection may be unstable. The AV cable and games are not included.
